Download and print frequently used advising forms by logging into my.rockhurst.edu and selecting the Intranet quick launch icon from the portal. Select the "Academic Advising" menu item from the left sidebar of the Intranet to access the following advising forms:
- Advising appointment checklist
- Class override request
- Closed class petition
- Class schedule worksheet
- Change of advisor request
- New student registration checklist
- Continuing student registration checklist
- Core requirements worksheet
Other frequently used forms can be downloaded from the Registrar site. These include:
- Official transcript request
- Pre-approval of transfer credit
