Judicial
Referral Form (11k)
To be used to verify completion of sanction when meeting
or doing an activity with someone other than the hearing
examiner. Sanctions will not be marked as completed
until sanctioned student turns signed form into Residence
Life.
FERPA waiver(16K)
To be completed by a student who wished to give the
University the permission to speak to someone other
than themselves about the educational record.
Off
Campus Drug/Alcohol Assessment Instructions (16k)
Directions and contact information for our off campus
assessment site. Sanctioned students are responsible
for contact the assessment site and making an appointment
before their sanction due date. Sanctioned students
are also responsible for providing a copy of the assessment
summary and a judicial referral form signed by the counselor
they saw.
