Athletics Participation Forms

Physical Exam
You must be examined by an American Medical Association (AMA) certified physician and complete a pre-participation physical exam after June 1, 2012, to be eligible for practice and competition during the 2012-2013 season. The physical forms are available on the Rockhurst website listed above. (Physicals conducted by a chiropractor will not be accepted.)
Insurance – Athletics Participation
Included in this mailing is a letter explaining athletics participation accident insurance carried by Rockhurst University through the NCAA and Mutual of Omaha. This is an “excess policy” that will only be used in the event of a bodily injury sustained during an authorized athletic event. This plan is in addition to your required primary health insurance coverage provided by you, your parents or as an optional purchase through Rockhurst.
Student-Athlete Move-in, First Practice Dates
Move-in for student-athletes participating in Soccer and Volleyball is mid-August. Student-athletes participating on other sports team may move-in with the general student body later in the month.
Student-Athlete Team Meetings
Each team will meet at the beginning of the school year for orientation and in-person completion of required NCAA forms and educational materials. We will also conduct a mandatory all student-athlete meeting in the Convocation Center, date and time TBD. Check back to this page regularly.
Expectations of Excellence:
- This is a great time in your life, so take full advantage of the opportunities Rockhurst has to offer. You’ll remember this time the rest of your life.
- Play hard and compete … 8 of our 11 teams participated in NCAA Championships since 2008.
- You represent yourself, your family, your team, your coach and Rockhurst; we will hold you accountable for your actions.
- Prepare for your life after college and after sports - study, go to class (not an optional activity). Use college athletics and your time at Rockhurst to get your degree.
Best wishes and GO HAWKS,
Gary Burns
